Special Issue Introduction

Over the past decades, the prognosis for patients with coronary artery disease has dramatically improved. However, the evolving profiles of these patients, which increasingly include multiple comorbidities and overall frailty, present new challenges in optimal management. This is especially true due to the overlap between ischemic and bleeding risk factors, such as age and chronic kidney disease. Therefore, a personalized approach to treatment is essential for optimizing the balance between risks and benefits. Modern strategies should incorporate not only traditional risk factors but also non-traditional ones, such as genetic data (e.g., polygenic risk scores), inflammatory markers, and coronary imaging, to enhance diagnostic accuracy, optimize therapeutic strategies, and improve both short- and long-term patient outcomes.

Interventional cardiology has historically been at the forefront of medical innovation, pioneering life-saving techniques such as percutaneous coronary intervention, transcatheter valve therapies, and new-generation stenting. These advancements have led to remarkable progress, but challenges remain in better predicting disease progression, anticipating procedural risks, and assessing treatment responses. Precision medicine offers a paradigm shift by focusing on individualized care through cutting-edge technologies, including advanced coronary imaging, genomic profiling, machine learning, and artificial intelligence.

This Special Issue aims to highlight the latest advancements and research that facilitate the integration of precision medicine into interventional cardiology. By exploring new methodologies and clinical applications, this issue seeks to contribute to the growing body of knowledge, enabling more personalized and effective interventions for cardiovascular diseases.
