Soft tissue management in the setting of injury can be considered in terms of two major conditions- management and replacement. The "management" of the soft tissue can be considered as maintenance or even protection of tissue in the acute setting. This often requires careful consideration of the debridement quality, what to debride, when to debride, and whether to debride or preserve. The historical concepts of radical debridement have been questioned in the setting of vacuum assisted closure, and appropriate debridement. Tissue replacement in the lower extremity is an evolving concept as well, with a broad variety of options based on the reconstructive ladder. We anticipate this Special Issue to offer technical tips and review of decision making in the setting of lower extremity injuries when deciding whether to remove, repair, or replace.
