Special Issue Introduction

Autologous breast reconstruction has undergone significant advancements over the last few decades, with numerous studies establishing its safety, reliability, and long-term patient satisfaction. As a result, it has become one of the preferred approaches of reconstruction for many patients seeking breast restoration. While abdominal-based flaps remain the gold standard for donor site selection, they may not always be available or sufficient to achieve the desired outcome. Therefore, the exploration of alternative options has become imperative. Over the years, several donor sites have been proposed, resulting in many flap options that significantly enhance the armamentarium of the reconstructive surgeons. As such, an individualized approach can be followed to accommodate each patient's medical history, body habitus, anatomy, and preferences to deliver an optimal result. In this Special Issue, we discuss the most up-to-date and innovative approaches to alternative autologous breast reconstruction, such as profunda artery perforator flaps, variations of upper gracilis flaps, lumbar artery perforator flaps, gluteal artery perforator flaps, as well as stacked flaps and multiple flaps. Hybrid approaches involving flaps and prosthetic devices will also be presented. In addition, we will review robotic surgery as it relates to minimally invasive autologous breast reconstruction.
